You will receive a hearing aid evaluation for $25 (regularly $75). Buy your hearing aid from Hendrick Hearing HealthCare and receive the Comprehensive Hearing HealthCare plan free of charge. This is a $550 value and includes batteries for the life of the hearing aid up to five years, yearly hearing aid evaluations, yearly audiometric screenings, quarterly hearing aid cleanings and adjustments and more. • Rehab Fitness Program Discount
A post-therapy program for those completing physical therapy or for those who want to maintain their fitness level. This program utilizes both and-based and pool exercises. • Lebed Method of Exercise – Focus on Healing Class Discount
The Lebed Method is a six-week therapeutic exercise and movement program for breast cancer survivors, people with any type of cancer or chronic illness or anyone looking for a fun method of group exercise. FirstChoice members receive a 10 percent discount on cash retail purchases. Current FirstChoice card must be presented at time of purchase. • Defensive Driving Classes
FirstChoice members can sign up for classes. The cost of the six-hour class is $10 for members, $20 for the general public and $25 for all ticket dismissals. Call the FirstChoice office to sign up for the next class.

• Free T-shirt
When new members enroll in FirstChoice, they receive a free T-shirt. T-shirts may be picked up at the FirstChoice office
weekdays during business hours.

• Personal Choices Brochure: Living Will and Medical Power of Attorney for Healthcare
Information on these important health issues is available for completion and will be kept in your medical record. You will receive the booklet when you are admitted or call the FirstChoice office at 670-2500 and one will be mailed to you.
