Texas Medical Board Notification

1. TMB Notification Letter received by physician.

2. Physician will notify practice manager who will complete the TMB form located below.

3. Risk Management will be automatically notified when the TMB smartsheet form is submitted.

4. Risk Management will contact physician to begin the response process.

5. Practice Manager or Risk Management will pull medical records and billing information.

6. Risk Management will provide a template letter and work closely with physician to provide a final response.

7. Once legal has approved the response letter, Risk Management will send the official letter to physician via DocuSign for signature.

8. Risk Management will then submit letter and supporting documentation to TMB via email/portal.

9. Physician and practice manager will notify Risk Management if any additional letters are received from TMB.
