Transitional Services

Transitional Services includes our 20-bed Skilled Nursing Facility (SNF), which serves patients who are transitioning from the acute care hospital to another level of care. Another important component of our Transitional Services program is ContinueCare Hospital at Hendrick Medical Center, a separate, fully licensed acute care "hospital within a hospital" that provides intensive and complex medical treatment for patients.

ContinueCARE Hospital at Hendrick Medical Center

ContinueCARE Hospital at Hendrick Medical Center is a 19-bed Long Term Acute Care Hospital (LTACH) located on the seventh floor of the Jones Building at Hendrick Medical Center. This "hospital within a hospital" is a separate, fully licensed acute-care hospital with the ability to provide intensive and complex medical treatment. Specialty programs are specifically designed to meet the needs of long-term, acute-care patients.

Skilled Nursing Facility (SNF)

Our SNF is located on the sixth floor of Hendrick Medical Center. All rooms are private, and a family room is available to allow more space to visit with your loved one. We have a therapy gym to assist our patients in becoming stronger to return to home or the community. Our SNF is unique in that we are located within a hospital which affords all the modern conveniences and technology to better care for our patients.

Conditions Treated

We provide sub-acute care for patients with diagnoses of, but not limited to:

  • Post joint repair/replacement
  • Non-complex post-operative care
  • Stroke
  • Diabetes
  • Congestive Heart Failure
  • Congestive Obstructive Pulmonary Disorder
  • Oncology related diagnoses
  • Wounds
  • Infectious Diseases

We provide 24-hour nursing care by nurses trained and skilled in a variety of areas.

  • Infusion/transfusion services
  • Wound care
  • Pain management
  • Teaching and training
  • Physical therapy
  • Occupational therapy
  • Speech therapy
  • Respiratory therapy
  • Radiation therapy
  • Case management
  • Pastoral care

For services on our SNF, a three-day acute hospital stay is required prior to admission to the SNF. This must have occurred within the previous 30 days.
