About FollowMyHealth

FollowMyHealth at Hendrick is an online tool that gives you access to your personal health records from any computer, smartphone or tablet:

FollowMyHealth at Hendrick helps our patients make more informed decisions through a secure, easy-to-use online portal. Patients can make more informed health decisions, set up proxy accounts for children and dependent adults, and receive email care reminders.

Getting Started

Connect via the mobile app

  1. Download the FollowMyHealth app from the App Store or Google Play.
  2. Once the app has downloaded, log in to your FollowMyHealth account or click Sign Up.
  3. Request connection with Hendrick Health.
  4. Search by the main Hendrick Health zip code, 79601 or search for Hendrick Health under “Hospital."

Connect via the web

  1. Go to hendrickhealth.org/followmyhealth.
  2. Access the FollowMyHealth login page.
  3. Then, choose “I need an account,” and follow the prompts to create your FollowMyHealth account.
  4. Once you have created an account, request connection with Hendrick Health.
  5. Search by the main Hendrick Health zip code, 79601 or search for Hendrick Health under “Hospital."

Frequently Asked Questions

What do I do if my connection request is rejected?
First verify that you have all required fields filled out accurately. Then check to see if you have included your Social Security Number. If edits are made to fit the requirements, please resubmit the connection request. If the required fields are already accurate, please contact patient relations at (325) 670-2915.

Will FollowMyHealth show me my lab results?
Yes. Lab results will show up in your patient portal with a connection to Hendrick Health for lab work done at one of our testing facilities.

Is there a fee to use FollowMyHealth?
No. FollowMyHealth at Hendrick is offered free of charge to all Hendrick patients.

Will I be able to set up an account for my children?
Individual accounts are not available to minor patients under the age of 18 years. To view a minor patient’s (newborn – 13 years of age) record, please set up a proxy account.

Minors age 14-17 are not eligible for a FollowMyHealth® account but can obtain a copy of their medical record from:

How do I set up Proxy Access?
Proxy access allows parents, legal guardians or caregivers to access a patient’s health information on the patient portal in FollowMyHealth®. Proxy access is an available option for adult patients (18 years and older) and minor patients (newborn – 13 years of age). Proxy access can be granted by the patient or their legal representative by completing the appropriate Minor or Adult proxy form for review and return to:

You will be contacted if additional documentation is needed for verification. Once verified, you will receive an e-mail to connect to the account requested.

Can I share my FollowMyHealth® portal information with someone else?
Yes, FollowMyHealth® does allow you to share your patient portal health information with another adult who has a FollowMyHealth® account from within your profile. Watch this interactive video for how to add an authorized individual to view your patient portal information.

This connection can be terminated at any time from within either account or by visiting HendrickHealth Information Management and providing a request to terminate connection. Notification of termination will be sent to both account portals for confirmation of termination.

I entered information into my patient portal; will my provider see it?
No, FollowMyHealth® shares information from your record to the portal. Information you enter will be for your tracking and documentation only.


How do I set up a 4-digit passcode for logging into the app?
Within your account settings, go to the Security section of your account and select the option to create a passcode.
