Dr. Chukwuemeka C. Ezeoke Departure Notice

Dr. Chukwuemeka C. Ezeoke will no longer be practicing with Hendrick Clinic Gastroenterology. Effective September 26, 2025, Dr. Ezeoke will transition to private practice at KeviMD Digestive Disease & Wellness Associates PLLC, located at 2650 Industrial Boulevard in Abilene.

Hendrick Clinic Gastroenterology remains committed to delivering high-quality healthcare and serving as your trusted healthcare provider.

To help with this transition, a Hendrick Clinic representative will contact patients to discuss their provider preference. If you wish to continue your care with a Hendrick Clinic provider, the representative will assist you in establishing care with one of our available providers. You may also view a current list of Hendrick Clinic providers at hendrickclinic.com.

If you prefer to seek care outside of Hendrick Clinic, please complete a Hendrick Clinic Medical Release form to release your records to that practice. Completed requests can be emailed to hendrickclinicROI@hendrickhealth.org or mailed to:
