Navigation to Survivorship Program

Hendrick Cancer Center is pleased to launch Navigation to Survivorship Program, which will ease the transition from cancer patient to cancer survivor.

This program will improve the patient's overall experience through support, education and advocacy. It is our goal to help our patients find their "new normal" and embrace life after cancer. By meeting the needs of the whole person, we hope to improve the patient's overall experience at Hendrick Cancer Center.

In the Navigation to Survivorship Program, a nurse navigator will assess each survivor for the following and will provide individualized care plans.

  • Fatigue
  • Lymphedema
  • Nutrition
  • Exercise
  • Genetic Testing
  • Religious and Spiritual Counseling
  • Psychosocial Assessments and Counseling
  • Community Agency Referrals
  • Financial Assistance
